By completing this form, you acknowledge that you will be contacted by one of our representatives regarding our products and services. Also, by way of this form, you consent to Progressa collecting using and disclosing your personal information as set out in our Privacy Policy. References cannot live at the same address and at least one must be a family member and all must be people you speak with regularly. In connection with our collection use and disclosure of your personal information, we may ask you to sign a further disclosure waiver or that you provide consent to a personal investigation Act in writing, including by electronic means.
The following documents may be required to process your loan.
‑ Completed Loan Application form with your signature;
‑ Proof of income – i.e. pay stubs.
‑ Bank statements showing last 90 days of banking transactions;
‑ Recent phone or utility bill (from the home address listed on application);
‑ Copies of two (2) pieces of I.D. – Government issued with photo and signature;
‑ Lease or Rental Agreement, Rent Receipt, proof of mortgage, or a signed statement from someone you are living with.

By submitting an application online for a loan with Progressa, you agree that the information you have given Progressa on the application is complete and accurate. You further agree that you have read our Privacy Policy and Terms of Use.
Please Read Before Signing: As a condition of Progressa lending me money based on the information provided above, I make the following representation: I am not contemplating filing for bankruptcy within the next 6 months and have not consulted with an attorney or trustee concerning bankruptcy in the last 6 months.

Please note that Progressa does not ever request a security deposit in exchange for processing a loan.
